Hi, Since the beginning of March 2018, Anthem has been denying claims billed for Individual and group therapy using rev codes 0914 and 0915. The denial states we need to submit a corrected claim with a more specific rev code or submit medical records. Since there aren't any other codes, we submitted medical records. Now we are receiving denials stating the facility is not licensed to provide those services Has anyone else been experiencing this and/or has anone found any resolution?
